| 内容記述 | 本研究の目的は高齢がん患者に看護師が行う終末期の意思決定の支援の実際と課題について明らかにすることである.調査は,がんに対する積極的な治療から緩和ケア中心の治療に方針を転換した(以下,ギアチェンジとする)高齢がん患者のいる施設に勤務し同意が得られた看護師15名に半構成的質問紙を用いた面接を行った.尚,本研究は岩手県立大学の倫理審査で承認を得ている.高齢がん患者のギアチェンジの実際において「家族が代理意思決定できるように環境と支援チームを調整する」「在宅に移行しにくい認知症患者でも,余命が限られていると家族は受け入れやすくなる」などの6カテゴリーが,意思決定の課題として「看護師が患者の代理意思決定における代弁者の役割を果たせていない」「代理意思決定をする家族の意思に患者の意見が反映されない」など3カテゴリーが抽出された.高齢がん患者の意思決定場面では、認知症の有無が家族の代理意思決定にも影響を与えていることが考えられた.また,高齢がん患者に関わる看護師の役割が患者の代弁者として倫理調整の役割を果たすためには家族に患者のケアや状況を説明する能力が求められていると考える. | |||||

| 内容記述 | We aimed to evaluate the actual state of and challenges in decision-making support by nurses for the terminal care of elderly cancer patients. We interviewed 15 nurses presenting consent at medical institutions, in which cancer patients (hereinafter called "gear change") had switched from positive treatment to mainly palliative care, by using semistructured questionnaires. The study was approved by the Ethics Committee of Iwate Prefectural University. The following categories were extracted. The actual state of gear change for elderly cancer patients included 6 categories such as "Changes in the environments and support teams for the patients' families allow decision-making on behalf of the patients" and "Even dementia patients who find it difficult to shift to home care become acceptable to their families if the patients have a limited life expectancy." The challenges of decision-making involved 3 categories such as "Nurses have not served as patient advocates in substitute decision-making for the patients" and "Patients' opinions have not been considered when the families took decisions on behalf of the patients." In the case of elderly cancer patients, the presence or absence of dementia also influences decision-making by their families. Moreover, nurses involved in elderly cancer patient care should explain patient care and conditions to their families to enable them serve as patient advocates of the ethical aspects. | |||||
