| 内容記述 | 岩手県の訪問看護師によるがん患者の在宅ターミナルケアの実態を明らかにすること、岩手県でがん患者の在宅ターミナルケアに携わる訪問看護師が行っている苦痛緩和のコツを明らかにすることを研究目的とした。尚,本論文でコツとは,看護を行う上で訪問看護師が工夫したり,心がけていることとする.在宅ターミナルケアの実態については,岩手県内のすべての訪問看護ステーション(2003年現在)53箇所に自記式質問紙を配布し33箇所(回収率62%)から回答を得た.がんのターミナル期にある患者や家族の看護にあたって,苦痛について日頃行っていることや工夫していることについての自由記述では,19箇所からの記述があり,類似した内容をカテゴリー化し11項目が抽出された.次に,同意の得られた13名の訪問看護師に対する半構成的面接法を用いた調査を行った.類似した内容を集めカテゴリー化した結果,苦痛緩和の「コツ」として,19カテゴリーが抽出された.面接調査から抽出された項目及び自由記述から抽出した項目から以下の18項目が訪問看護師の行う苦痛緩和のコツとして抽出された.・医師との連携・症状緩和ケア・鎮痛剤使用の指導・そばにいること・日常生活を維持する援助・タッチによる心地良いケア,気分転換による心地良い時間の確保・病状受け入れへの支援・患者と家族の意思の尊重・看護師の対象者の立場で考える姿勢・コミュニケーション(自然体で話す,傾聴,言葉かけ)・患者や家族への情報提供・家族指導・相談・患者や家族の支援・励まし・家族間調整・遺族ケア・社会資源の活用・スタッフのストレスの軽減をはかる訪問看護ステーションの体制・チーム連携 | |||||

| 内容記述 | To clarify special nursing skills for relieving pain in visiting nurses performing terminal care for cancer patients at home in Iwate Prefecture. Methods : A self-administered questionnaire was mailed to all 53 visiting nurse stations in Iwate. Valid responses were obtained from 33-stations. The questionnaire included a section where respondents could freely describe nursing skills routinely used to relieve pain. Semi-structured interviews with 13 consenting visiting nurses were conducted and transcribed. Results : Based on interview and questionnaire results, 18 special nursing skills for relieving pain among visiting nurses were extracted : Collaborate with physicians , - Provide care to alleviate symptoms, - Provide education regarding use of analgesics, - Presence, - Assist with maintaining ADL,- Provide comforting care through touch and psychological diversion, - Support patients to accept their condition, - Respect the opinions of patients and family members, - Think from the perspective of caregivers, - Communicate effectively, - Provide information to patients and their families, - Educate and consult with family members, - Support and encourage patients and family members, - Act as an intermediary among family members, - Bereavement care, - Utilize social resources, - Reduce stress of visiting nurse, - Collaborate with team members | |||||
