| 内容記述 | 日本の地域医療が今や健全でないことは誰の目にも明らかである。本稿の目的は国と地方と住民(患者)の関係に着目して、過疎地域における医療問題と自治体(病院・診療所)の役割を意識しながら、地域医療の根源的な問題を明らかにし、その課題を検討するための素材を得ることである。地域医療問題の根源は(地域)医療改革が国・地方の行財政コストの抑制(経済効率化)のための改革、患者(住民)不在・軽視の改革にあることを暫定的に提起したい。したがって、いわば目的と手段の逆転を解消し、患者主役・重視の改革を行なうのである。これを踏まえると、地域医療の基本課題の一つに保健、医療、介護(福祉)の総合化があげられる。 | |||||

| 内容記述 | It is clear that sound health care administration is no longer maintained in prefectures, towns and villages of Japan. As such, the objectives of this article are to identify fundamental issues in community health care in Japan focusing on the relationship between central government, local governments, and citizens (patients) while being aware of the health care issues in depopulated areas and the role of local governments (municipal hospitals and clinics), and to obtain data for the study of such issues. In this article, I will propose a hypothesis that the fundamental problem in community health care rests in the undergoing health care reform that focuses on reducing administrative and financial costs of central and local government (to improve economic efficiency) while ignoring patients (citizens). In line with this hypothesis, central and local government should therefore implement reforms in which patients are emphasized and play a central role, thereby eliminating the contradiction between objectives and means. Based on the above discussion, the integration of public health, health care and nursing care (welfare) can be raised as a basic community health care issue. | |||||
