| 内容記述 | 市町村合併に伴う独自制度の廃止・後退はその意義を根本的に問うことになる。本稿の目的は、2005年に誕生した岩手県西和賀町の旧町村(沢内村と湯田町)を研究対象にし、旧沢内村「老人医療費10割給付制度」に焦点を当て、両地域の合併論議におけるその取扱いを分析することによって、(1)それが終焉に至るプロセスを明らかにすることである。(2)市町村合併問題さらに地域医療行財政問題への含意を検討することである。老人医療費10割給付制度は合併協議会や旧湯田町議会において主として財政負担の面から議論され、沢内村長は早々に患者一部負担の導入を余儀なくされ、給付制度のあり方に関して防戦一方になり、また、沢内議会においては村長との議論が著しく不足し、村民が置き去りにされた印象を拭えない。老人医療費10割給付制度の取り扱いに関する分析から次のことが示唆される。(1)独自制度(・政策・サービス)の維持は非常に困難であること。(2)患者一部負担の水準を合理的に決定しにくいこと。(3)むらづくり(生命行政)に位置づけて議論すること。(4)国の生命行政に対する財政的責任が問われていること。 | |||||

| 内容記述 | The abolition or regression of systems unique to a municipality that takes place in conjunction with a municipal merger leads to fundamental question being raised regarding the mergers' significance. The purpose of this paper is to (1) clarify the process leading to the termination of such system in merged municipalities and (2) examine the implications of that process with regard to issues with municipal mergers as well as to financial and administrative operations for community health care. In this paper, the case of Sawauchi village and Yuda town, a village and town in Iwate Prefecture which were merged in 2005 forming Nishiwaga town, is examined through the analysis of discussions held by the councils of these municipalities regarding their merger, with a focus on the 100% Medical Expenditure Benefit System for the Elderly which existed in Sawauchi village. This benefit system was deliberated by the merger council and the assemblies of Yuda town mainly with regard to the financial burden of operating the system. The mayor of Sawauchi village was quickly forced to introduce a patient copayment system and had to continuously make efforts to defend his position regarding what he considered the ideal from of the benefit system to be. In addition, there is a strong impression that discussions with the mayor in the Sawauchi Village Council were extremely insufficient and that the people of the village were abandoned. The results of the analysis of the 100% Medical Expenditure Benefit System for the Elderly suggest the following: (1) it is extremely difficult to maintain a municipal system, policy, or service which is unique after a merger, (2) determining a reasonable level of patient copayment is difficult, (3) discussion of such systems, etc. should be made from the viewpoint of revitalizing town and villages (and seimei gyosei), and (4) the financial responsibility of the central government regarding seimei gyosei is in question. | |||||
