| 内容記述 | 本研究の目的は,造血幹細胞移植の準備期にある患者の気分の経時的な変化を日本語版POMS,日本語版MAC,自尊感情尺度を用いて評価し,気分の変化と移植に関する認識の違いを記述することである.調査は,移植前4週間頃,移植前2週間頃,移植直前の3回行った.研究協力の承諾が得られた移植予定の患者13名に対し,POMSなどの尺度による調査と,移植に関する認識について尋ねた半構成的質問紙を用いた面接調査を行った.POMSの各時点における平均を一元配置反復測定分散分析により検討した結果,下位項目の活気[F(2,24)=4.525,p=0.027]と疲労[F(2,24)5.926,p=0.008]で有意差が認められた.これら有意な平均の変化が見られた下位項目の得点変動に基づいて対象者を2群に分け,移植に関する認識の内容を両群間で比較した.活気不安定グループは医療者などへの依存的な認識が多く,活気安定グループは客観的な認識がみられた. このことから移植前に患者は移植を自己決定しても,活気不安定グループは依存的な認知があると考えられ,特に活気などの気分に変化のある患者には移植前からの正しい認知についての患者教育としての看護援助が必要であること示唆された. | |||||

| 内容記述 | The aim of this study is to describe the recognitions of transplantation and mood changes with time in patients during the preparation period of hematopoietic stem cell transplantation. We investigated the patients' recognitions and mood changes at three different times: around four weeks before transplantation, around two weeks before transplantation, and immediately before transplantation. Using semi-constitutive questionnaire forms investigating the Profile of Mood States (POMS; Japanese version) etc and patients' recognitions of transplantation, we conducted an interview survey of 13 patients scheduled for transplantation who had consented to be involved in the research. The average scores of each POMS item were analyzed using oneway repeated measures analysis of variance, and there was a significant difference between the items Vigor [F(2,24)=4.525, p=0.027] and Fatigue [F(2,24)=5.926, p=0.008]. The subjects were divided into two groups on the basis of score changes in sub-items showing significant changes in the average, and the recognitions of transplantation were compared between the two groups. The unstable vigor group frequently had dependent recognitions of people playing active roles such as medical professionals, and the stable vigor group had objective recognitions. It was considered that even if patients had chosen to receive the transplant, the unstable vigor group had dependent recognition.
