| 内容記述 | 本研究の目的は,岩手県内の学校に在籍している糖尿病の児童生徒への養護教諭のかかわりの実際や,それに伴う問題点を明らかにすることである.L.W.GreenによるPROCEEDモデルと兼松の小児糖尿病管理におけるアプローチを基盤とした概念枠組みにより,養護教諭14人に対して糖尿病の児童生徒16人の学校生活でのかかわりについて半構成的面接を行ない,語られた内容を質的に分析した.その結果,前提要因で糖尿病の児童生徒が『糖尿病の知識はある』と判断した養護教諭は,実現要因で『配慮』『確認』を行ないつつも,糖尿病の児童生徒の療養行動を認めた『自己管理に任せる』かかわりを行ない,強化要因へのかかわりは少なかった.一方,前提要因で糖尿病の児童生徒が『糖尿病の知識は不十分』と判断した養護教諭は,実現要因で『配慮』『確認』『声かけ』『指導』のかかわりを,強化要因で『担任』『家族』といった関係者との『連絡』『相談』のやりとりを行なっていた.養護教諭の糖尿病児童生徒へのかかわりの問題点には,養護教諭の糖尿病に対する認識に関するもの,糖尿病の児童生徒へのかかわりの難しさ,強化要因へのかかわりの難しさがあった.養護教諭は,前提要因である糖尿病の児童生徒がもつ『糖尿病の知識』や『気持ち・願い』をよく把握した上で『自己肯定感』を高めるようなかかわりを行ない,教職員へかかわりの『方針』を示していく必要がある.さらに養護教諭には,糖尿病の児童生徒のかかわりや2型糖尿病予防に関する相談ができるシステムとして,糖尿病看護に精通した者や糖尿病療養指導士からの支援体制の構築が必要である. | |||||

| 内容記述 | The purpose of this study was to clarify the conditions, and the related concerns of school nurses attending to schoolchildren with diabetes who are enrolled in schools in I Prefecture. Based on the "Proceed" model by L. W. Green, and the methods of Kanematsu, regarding the management of juvenile diabetes as a conceptual framework, semi-structured interviews were carried out with fourteen school nurses concerning sixteen schoolchildren with diabetes, to whom they were attending about school life. The results were that school nurses determined schoolchildren with previously-known causes of their diabetes as 'having a knowledge of diabetes' and, despite the nurses acknowledging the actual main cause by 'attention' and 'confirmation', 'left to self-management' the procedures for the medical treatment of the diabetes and little was carried out by them concerning the regulation of the main causes. In contrast, when school nurses determined schoolchildren with diabetes, the causes of which were likewise previously-known, as 'having an insufficient knowledge of diabetes', they acted with respect to the actual main cause by 'attention', 'confirmation', 'addressing the schoolchildren about it' and 'guidance' and, concerning the regulation of the main causes, 'communication' and 'consultation' was carried out with such related people as the 'homeroom teacher' and 'family'. The concerns of school nurses attending to schoolchildren with diabetes were the difficulties of attending to schoolchildren with diabetes and difficulties concerning the regulation of the main causes. It is necessary for school nurses to act so that schoolchildren with previously-known causes of diabetes will have 'knowledge of diabetes', will have a better understanding of 'feeling and needs' and, in particular, to act to develop a 'self-positive attitude' in the schoolchildren, and to demonstrate a 'policy' regarding teachers. Furthermore, the creation of a support system regarding schoolchildren with diabetes and the prevention of type 2 diabetes, involving diabetic nurses, well-informed people and professionals in the treatment of diabetes, is necessary. | |||||
