 note = {痴呆対応型共同生活介護事業(以下、グループホームとする)は、介護保険法施行後急速に増加している.グループホームの利用者は、介護保険施行当初は軽度から中等度の痴呆性高齢者が中心と考えられてきたが、年数を重ねるにつれて痴呆が進行し、重度化する状況にある.そのため、グループホームにおけるターミナルケアについて考えていくことが重要となっている.そこで岩手県におけるグループホームでのターミナルケアの取り組みについて現状を把握し、課題を明らかにすることを目的として調査を行った.調査協力の得られた23ホームのホーム長に対して半構成的面接法による調査を行った結果、グループホームでターミナルケアを行うにあたって、以下の4つの課題が見出された.1.スタッフの質的問題 2.医療面での問題 3.建物の構造上の問題 4.家族の理解の問題また、グループホームにおけるターミナルケアの必然性について、議論が不足し論点が整理されないままの現状においては、今回の調査で見出された課題への対応を含め、ホーム利用者のターミナル期について、個々のグループホームにおける考えにより決定されていた.グループホームにおけるターミナルケアについて、グループホーム全体でもっと真剣に論議され、全体的な合意がなされるような方策が早急にとられなければならないことが示唆された., The number of group homes for dementia patients has increased rapidly after the enactment of the nursing-care insurance law. At first, most group home users were elderly patients with mild to moderate dementia. However, over time, the severity of dementia in group home users has increased. Terminal care in group homes is thus starting to become a relevant issue. The present study was conducted to ascertain the current state of terminal care in group homes in Iwate Prefecture, and to clarify problems associated with terminal care in group homes. Semi-structured interviews were held at 23 group homes, and the following 4 problems with terminal care in group homes were identified : 1. Problems with substance of care staff 2. Problems with medical care 3. Problems with building structures 4. Problems with family understanding Currently, little discussion about the necessity of terminal care in group homes has been initiated, and related issues have not been identified and organized. As a result, terminal care for group home users, including countermeasures for problems identified by the present study, is determined based on the individual approaches of each group home.
